Heart2Heart Companions is a volunteer-driven initiative dedicated to enhancing the quality of life for residents in skilled nursing facilities across California. This program matches compassionate volunteers with skilled nursing residents to provide much-needed companionship and support for their social, mental, and emotional well-being. Recognizing the isolation and mental health challenges many residents face, Heart2Heart Companions offers a bridge to the outside world, fostering meaningful relationships that positively impact residents' overall health and happiness.

Volunteers engage residents in various supportive activities, including friendly conversations, recreational activities, reading, and arts and crafts, based on the residents' preferences and needs. 

This personalized approach not only combats loneliness and depression but also empowers residents with a sense of belonging, dignity, and purpose.

Eligibility is open to all skilled nursing residents in California, with availability dependent on the number of active volunteers. Heart2Heart Companions aims to create a scalable model, continually recruiting and training volunteers to extend this service to as many residents as possible, ensuring no one is left without the companionship and care they deserve.

  The Resident Essentials Fund at Hands of Agape is designed to provide skilled nursing residents in California with gifts and personal supplies that promote comfort, dignity, and well-being. This program ensures that residents have access to essential items that enhance their quality of life, such as personal care products, clothing, and other necessities that may not be covered by insurance, family, or personal resources.

The program is open to all skilled nursing residents who need these items, with eligibility determined based on available funding. The Resident Essentials Fund aims to foster a sense of individuality, comfort, and self-care, helping residents feel more at home and valued in their environment by offering a range of personal supplies. Hands of Agape seeks to provide a vital resource that enhances the physical and emotional well-being of skilled nursing residents across California through this initiative.

  Care Beyond Coverage is a program designed to provide financial assistance to skilled nursing residents in California who require custom medical equipment and other essential supplies that are not covered by insurance, family, or personal resources. This program aims to alleviate the financial burden on residents, ensuring they can access the necessary tools and equipment to improve their quality of life and comfort.

Eligibility is open to all skilled nursing residents in California who need these critical items, with assistance provided based on available funding. By filling the gaps left by traditional insurance coverage, Care Beyond Coverage ensures that residents receive the equipment and supplies they need for proper care, mobility, and daily comfort. This program supports the dignity and well-being of residents by providing vital resources that help them live more comfortably and independently.

 

  Community Connection: Enrichment Outings is a program designed to enhance the quality of life for skilled nursing residents by providing opportunities to engage with the community outside of their care facility. Through transportation provided by Hands of Agape company vehicles, residents who are physically and mentally able will be taken on enriching outings to a variety of destinations, including parks, malls, zoos, festivals, and other local attractions.

This program aims to promote social interaction, physical activity, and emotional well-being by offering residents the chance to explore new environments and participate in activities that stimulate mind and body. It provides a refreshing change of pace and encourages connection with the broader community, fostering a sense of independence and joy.

Eligibility for participation is open to all skilled nursing residents in California who are physically and mentally able, with participation subject to available funding. Through Community Connections, residents will enjoy the freedom to experience the world beyond their facility in a safe, supportive, and enriching way.
